I confirm that I am the parent/legal guardian of the registered child and consent to their participation in the Miftaah Juniors Slick City event on October 31, 2026.

I understand and agree that: This is NOT a drop-off event. A parent/legal guardian must remain on-site and supervise the child throughout the event.

I am responsible for my child's supervision, behavior, and compliance with all venue safety rules. I acknowledge that the activities involve inherent risks, including potential injury. Miftaah Juniors is organizing this outing but is not providing individual childcare or supervision.

Slick City's separate waiver is required, and instructions will be shared before the event. I have read, understood, and agree to the above terms.
